Additional Forms If you already own a copy of Healthy Habits then you know that this book is filled with helpful forms that allow you
to easily and conveniently keep track of the foods you eat and the commitments you make. You can download extra copies of the "Weekly Food Intake Form" here. Be sure to
print on both sides of the paper so that you'll have space to track an entire week's worth of information on a single sheet of paper. This single sheet then folds easily so you can carry it with
you.
Viewing Instructions:
In order to view any of the forms, you must obtain a copy of Adobe Acrobat Reader. This is a free program available online from Adobe. Simply click on the image below,
download and install the
reader, and you'll have all the necessary software needed to view the sample chapter.
Once you have Adobe Acrobat Reader installed (or if you already had it in the first place), click on the form that you would like to view below. If you would like to download the file to your computer
for later viewing, right click on the form, select "Save Target As...", specify the location to save the file to, and you're good to go.
